One of the biggest inefficiencies in our company is our issue tracking software. It’s hard to enter issues into, it’s hard to categorize issues, it’s hard to search the issue database, and it’s hard to communicate the issues to others. All in all, it generally leads to confusion and tons of meetings.
Are there any good products that are commonly used in the chip design community?
Janick’s “Writing Testbenches” book highlights some of the same problems I see with our system and also makes suggestions for what a good product would include.
Some things I’m particularly interested in are:
Web and email interfaces for entry and reporting.
Issue fields should have a way of allowing the user to easily recognize if the field is required and also have a list of possible values if necessary.
Easy way of searching the fields, saving the searches, and sharing the search technique with others.
Easy way to send emails to people regarding the issue.
Easy ways to create a new issue, like starting with an issue template, or basing the current issue off a previous issue. This may fill in some of the fields for the user.
A good editor that can create links, do syntax highlighting, create bullets, etc..

In our company, we use bugzilla as our main bug reporting/tracking software.
Check it out its free and it works out well for us:

| You could also try RT, by www.bestpractical.com. We use this free tool for in-house tracking, and it works well. |
